The Medical Home Project (MHP) celebrated its 20th anniversary at Lehigh Valley Hospital\u2013Cedar Crest, part of Jefferson Health, recognizing two decades of work to improve care for people with disabilities. Founded by Sweety Jain, MD, the group has grown from five members to more than 20. The celebration opened with remarks by Robert Murphy, MD, Jefferson Health\u2019s Enterprise Chief for Provider Network Development, and featured speakers from Jefferson\u2019s FAB Center for Complex Care, Children\u2019s Hospital of Philadelphia, internal and family medicine residency programs, and patient self-advocates. A founding member spoke about the value of community collaboration, and artwork by individuals with disabilities was displayed.<\/p>\n
